Non-Invasive Cosmetic Treatments

We are born with a finite number of fat cells which increase in size to store fat globules with weight gain. Cryolipolysis is a treatment for permanent elimination of fat. By placing vacuum applicators onto pockets of fat, tissue is selectively frozen down to -8 degrees. At this temperature fat cells may be destroyed, and subsequently excreted by the body over a period of 6-12 weeks.

As tissue is not penetrated or broken during cryolipolysis, patients often able to return to normal activities immediately after treatment.

Although useful for targeting pockets of fat, cryolipolysis should not (and cannot) be used as a treatment for obesity. Instead it is suitable for use alongside exercise and a healthy lifestyle.

FAT FREEZING (CRYO) FAQs

Most people are suitable for cryolipolysis however you may be declined treatment in the setting of the following:

  • age < 18 years
  • pregnancy and breastfeeding
  • systemic inflammation or illness including hepatic dysfunction, metabolic disorders, diabetes and/or hepatitis
  • concurrent use of blood-thinning agents aspirin, clopidogrel (plavix), warfarin, heparin etc
  • tissue composition not suited to fat cryolipolysis
  • conditions that cause sensitivity to cold e.g. paroxysmalcold haemoglobuinuria, cyroglobulinaemia, cold urticaria, reynaud's disease
  • previous trauma or surgery to the area including in cases of excessive scar tissue formation
  • pre-existing or suspected hernia in the area of treatment
  • underlying neuropathy or neuropathic disorder
  • disorders of lymphatic insufficiency
  • conditions of vascular insufficiency e.g. peripheral vascular disease
  • clotting and/or bleeding disorders
  • active, or history of underlying skin condition at the site e.g. psoriasis, eczema, infection, open lesions
  • implanted medical devices e.g. shunts, pacemaker, defibrillator
  • smoking and/or recent alcohol intake

+ What are the risks and expected side effects?

Immediately following cryolipolysis:

  • Redness, bruising, swelling and pain in the treatment area is expected and may persist for multiple weeks to be replaced by itchiness, sensitivity and/or numbness. Generally the latter-mentioned symptoms will resolve within a matter of weeks-months
  • Following resolution of swelling hardening and lumpiness at the treatment site may occur. This can take over a month to resolve
  • During and after treatment of fat under the chin, a feeling of fullness in the throat may be experienced. This may create the sensation of difficulty swallowing

Less commonly the following adverse effects may occur:

  • Intense muscle cramping/spasm or shooting pain lasting 1-2 weeks, may be managed with simple analgesia and/or compression garments
  • Vasovagal (fainting symptoms) following removal of applicators - mostly due to redistribution of blood flow that occurs with warming of frozen tissue
  • Hyperpigmentation - often resolves spontaneously
  • Hernia - may occur as a result of treatment due to intense suction applied by the vacuum. A thorough assessment of underlying hernia risk will occur during consideration of your suitability for treatment
  • Paradoxical hyperplasia - may develop 2-5 months after treatment and results in rebound exacerbation of fat tissue
  • Delayed nerve pain has been associated with cryolipolysis treatments. Most often transient, this can be managed with medication targeted towards the treatment of nerve pain
  • Poor results - can include assymmetry, poor response to treatment and/or demarcation of tissue

+ How many treatments will I need?

Generally a minimum of 2 applications at each site of concern is recommended at 2 months apart. However this is extremely variable between individuals, dependent on tissue composition, response to treatment and the site targeted.

Complete resolution of fat is often not achievable, however improvement may be possible. Studies have shown an average reduction of between 20-30% of the fat layer per application.

+ What should I do prior to treatment?

  • Ideally avoid use of blood-thinning medication (in accordance with medical advice) for 2 weeks prior to treatment
  • Avoid alcohol consumption and/or recreational drug use at least 48 hours prior to treatment
  • Commence an exercise and dietary program conducive to optimal weight management
  • Consider cessation of smoking

+ What happens during treatment?

During treatment you will discuss the risks and side effects of treatment with your artisan; prior to being provided the opportunity to ask questions and sign a consent form. Photos will be attended, and the treatment area(s) identified. Your weight and measurements including skin fold will be taken and the areas prepared for treatment.

The location of the area of concern will be confirmed with you prior to proceeding.

You will be positioned on our treatment bed and gel pads will be applied to the treatment site(s). Applicators are applied and you will experience the sensation of the vacuum, coupled with cooling. Approximately 5 minutes into treatment the area will go numb. You will remain on the machine between 45-70 minutes depending on the treatment, during which you will be offered our ipad to watch subscription streaming. Throughout the procedure your artisan will continue to enquire as to your comfort. Refreshments are available if required.

Once the cycle(s) is complete, 2 minutes of massage is attended which can become temporarily uncomfortable as tissue defrosts.

+ Is it painful?

Cryolipolysis is associated with some discomfort experience for most patients. Depending on the individual and the area treated, intense pain may be experienced following removal of applicators and attendance of massage. This experience is generally only temporary, often lasting less than 5 minutes and does not occur with every patient. Following treatment, the area(s) may feel swollen, bruised and tender. Simple analgesia may be self-administered (e.g. paracetamol) as advised by your artisan.

+ Aftercare

Prior to leaving the clinic, your artisan will provide you with printed aftercare information, however generally following treatment you should:

  • Sleep with the area elevated as possible
  • Avoid direct sun exposure of the area and wear spf 30+ daily
  • Avoid use of physical exfoliants or active skin care at the area for at least 72 hours
  • Avoid intense exercise/activity for at least 48 hours post treatment, as well as excessively hot showers + saunas which may exacerbate swelling and bruising. After this time regular, gentle activity and massage is encouraged to facilitate lymphatic drainage
  • Avoid long distance travel and wearing tight/uncomfortable clothing that may impede lymphatic drainage
  • Simple analgesia (paracetamol, ibuprofen) may be used for discomfort and swelling in conjunction with physician advice
  • In the weeks following treatment a low sugar, low salt diet accompanied by good intake of water is encouraged to avoid fluid retention
  • Hirudoid cream, arnica (topical or oral) and bromelain may be additionally used post treatment to expedite recovery

  • smoking and/or recent alcohol intake

+ What are the risks and expected side effects?

Immediately following treatment:

  • Redness, warmth, tingling, bruising, swelling and pain in the treatment area is expected which may itch in the days following. Intense swelling can cause downtime of 3-7 days and the site is likely to feel tender during this time. Ice may be used to reduce discomfort.
  • Following resolution of swelling hardening and lumpiness of this site it common. This can take over a month to resolve

Less commonly the following adverse effects may occur:

  • Numbness - this may persist for over a month following treatment though in most cases fully resolves
  • Infection - all care is taken to ensure no introduction of bacteria during treatment. Strict adherence to post care recommendations is advised to reduce risk of infection thereafter
  • Mandibular marginal nerve palsy and/or injury - leading to asymmetry of smile. This is a rare reaction and in almost all cases has resolved with time
  • Skin/muscle ulceration and damage - can occur when product is placed too superficially or too deeply. Often the side effects can be conservatively managed with appropriate wound care
  • Alopecia (hair loss) at the site
  • Difficulty swallowing can be a result of deep nerve injury and/or due to profuse swelling
  • Tissue fibrosis is not uncommon and can be felt as areas of hardened/non elastic tissue in the treatment area. This may cause visual deformity in severe cases though may be reduced with appropriate management
  • Poor results - can include asymmetry and/or poor response to treatment. In 1% of the population, resistance to treatment exists and results may be unachievable.

+ How many treatments will I need?

Generally a minimum of 2 treatments at 8-10 week intervals is recommended in order to see visible effects. However some people may require in excess of this depending on individual variables and response to treatment.

Complete resolution of fat is often not achievable, however improvement may be possible. Often in the case of submental treatment, improvement by 1 grade on the double chin assessment scale is likely.

This area will be prepared for treatment and gridded accordingly.

Treatment will proceed by placing small aliquots of therapy into the treatment site which is expected to swell significantly soon after commencement. Swelling may be accompanied by mild discomfort which can somewhat be alleviated with a cool compress. Throughout the procedure your artisan will continue to enquire as to your comfort.

Following treatment the site will be re-cleansed and topical antiseptic applied.

+ Is it painful?

Treatment is mildly uncomfortable and initiates inflammation immediately upon placement into tissue. This can be accompanied by a hot, burning, prickly sensation which can be unpleasant. This sensation can persist for days though often dulls to more of a swollen, tender feeling. Ice is recommended post treatment to assist comfort and recovery, and unless otherwise advised by your doctor, simple pain relief (such as paracetamol) may be taken prior to help reduce pain.

OVERVIEW

Radiofrequency microneedling involves the application of multiple fine pins charged with radiofrequency to the skin. Performed mechanically, and in a controlled setting, the depth and intensity of treatment can be varied to provide fractionated tissue ablation that is suitable for most skin types. The aim is collagen generation and replenishment, subdermal remodelling, closure of superficial fine blood capillaries and turnover of outer tissue layers.

Morpheus 8 aims to offer the traditional advantages of RF microneedling, at high intensity. This means it is capable of addressing a wide variety of indications. In addition to uniquely being able to treat to a depth of 7 mm. The Morpheus 8 has an effective range that extends an additional 1 mm into the subdermis, and is able to deliver stacked pulses. Multiple energy bursts aim to target not only the outer skin layers to treat texture, pigmentation and fine lines, but may also effect and destroy adipose tissue making the Morpheus 8 a suitable option for addressing problem areas such as the jowls, neck, abdomen, inner arms and thighs.

Crusty areas of the skin can emerge, along with redness and continued swelling. The skin will likely feel tight and, as the MENDs continue to come to the surface of the skin it will feel sandy and dry. This is all quite normal and will follow quickly with flaking and peeling which helps slough the MENDs off of the skin. Mends will dissipate in 2-4 weeks on the face and 4-6 weeks on the body

Less commonly the following adverse effects may occur:

  • Skin Tone changes (light or dark spots on the skin) lasting 1-6 months
  • Failure to achieve the desired result
  • Allergic reaction to any of the products used during treatment
  • Eye injury for treatment around the eye are - though this is very unlikely

+ How many treatments will I need?

Depending on your concern, multiple treatments are often required, however changes may be seen following the initial session. During your pre-treatment consultation, your artisan will determine the number of suggested treatments. This is dependent on factors such as your skin type, area of the body being treated and your aims; though generally speaking, 1-3 sessions are required at intervals of 3-6 weeks for high intensity settings, and 2-4 sessions every 3-6 weeks at lower settings.

It is also recommended that you return for maintenance sessions every 6-12 months to maintain the initial results.

+ What should I do prior to treatment?

  • PLEASE DO NOT WEAR MAKEUP TO YOUR APPOINTMENT
  • Avoid exposing the area to the sun or applying fake tan in the two weeks prior to treatment
  • Consider paracetamol 30 minutes prior to RF microneedling if your doctor has not advised otherwise
  • Cease use of actives such as vitamin A/retinol based skin care in the area at least 3 days prior
  • Avoid alcohol consumption 48 hours either side of treatment
  • Consider cessation of smoking
  • Cease any anticoagulant medication (aspirin, warfarin), anti-inflammatories and supplements that thin the blood (in consultation with your doctor) at least 2 weeks prior to treatment
  • If you have a history of cold sores at the treatment site, preventive medication is often recommended and is essential in cases of frequent or severe outbreaks

+ What happens during treatment?

During treatment you will discuss the risks and side effects of treatment with your artisan; prior to being provided the opportunity to ask questions and sign a consent form. Photos will be attended, and the treatment area(s) identified.

This area will be prepared for treatment and a numbing agent applied as necessary. When you have had sufficient time for the numbing agent to work, skin will be cleansed with an antibacterial wash.

Treatment will proceed and your artisan will continue to enquire as to your comfort. During this time it is important to try and remain still. Treatment will continue until the skin is red and slightly swollen, or there is minor pinpoint bleeding observed. This may be accompanied by a tingling heat sensation.

Following treatment the site will be re-cleansed, and post laser cream + SPF applied.

+ Is it painful?

The Morpheus 8 is an uncomfortable treatment that is made more tolerable with topical and local anaesthetic. To further aide your comfort, paracetamol may be also be taken within 30 minutes of treatment. We do not recommend aspirin, diclofenac (voltaren), meloxicam, ibuprofen (nurofen) or any other anti-inflammatory prior to treatment as they are likely to exacerbate swelling and bruising and may reduce efficacy.

A normal experience is that of heat, pressure and an electrical pulse during treatment, with some areas more sensitive than others. However please advise your artisan if pain is excessive as the treatment can be customised.

+ Aftercare

Prior to leaving the clinic, your artisan will provide you with printed aftercare information, however generally following Morpheus 8 Radiofrequency Microneedling:

  • You should avoid adding excessive heat to the area (saunas, hot showers, excessive exercise, sun exposure, excessive sweating) for 24-48 hours
  • Avoid submersion of the area/s (pools, spas) for at least 1 week post treatment
  • Apply ice as required for comfort
  • Sleep with head elevated to aid resolution of swelling
  • Avoid direct sun exposure and wear spf 30+ daily
  • Avoid use of physical exfoliants, active skin care and/or exposure to extreme temperatures, friction and irritation
  • Avoid chemical peels, microdermabrasion, IPL/laser, or intense skin treatments/cosmetic intervention for at least 2 weeks
  • Apply post laser cream, aloe vera, bland moisturiser, emollient cream, antibiotic cream and/or occlusive dressings to the treated area until symptoms resolve. In the first 3 days ensure you following the skin care recommendations of your artisan - this may be antibiotic cream in the case of acne treatment
  • Avoid active skin care (acids, retinol/vitamin A) and perfumed creams/moisturisers for 7 days
  • Antihistamines may be used to aid itchy skin in healing phase
  • Avoid touching the treatment areas until all mircowounds have resolved and inflammation subsides
  • Mineral makeup is advised if you require make up immediately after treatment, otherwise no make up should be applied for 48 hours

TIXEL OVERVIEW

SKIN RESURFACING | LASER RESURFACING | SKIN REJUVENATION

Tixel is a novel technology that uses a titanium tip at high temperature to cause thermomechanical injury to the skin and vapourisation of surface micro channels. The effect of this may be to cause skin resurfacing and rejuvenation, and encouragement of collagen generation. Furthermore, channels created via the process may be utilised for infusion of multiple active products which may help with reducing pigmentation, flushing, hydration, acne etc.

The power of the Tixel can be manipulated be comparative with that of a CO2 fractionated laser, or kept gentle enough to treat melasma without little risk for rebound pigmentation. As Tixel does not employ a laser or light source, it is also able to treat directly over the eyelid, making it one of few options available for non surgical rejuvenation of the eye area and hooded eyelids.

  • If you have a history of cold sores, a prophylactic anti-viral may be recommended

Skin needling is controlled injury of the skin induced by multiple fine needles to a depth of 0.1-2.5mm. Such injury is thought to induce wound healing and via that process promote the formation of collagen. In addition, skin needling may force renewal of tissue and may therefore be employed to treat multiple skin concerns. Because skin needling does not involve the addition of heat or light, the treatment is generally indicated for treatment of darker skin, and skin types prone to pigmentation. It may assist with resurfacing, pigmentation, fine lines, dullness, scarring and textural irregularities.

  • Skin may be more prone to sun damage
  • Visible peeling is expected

Less commonly the following adverse effects may occur:

  • Exacerbation of acne and/or underlying skin conditions may occur
  • Post inflammatory hyperpigmentation - is rare though can occur following treatment. If occurring, it is usually transient and can be supportively managed
  • Treatment failure, or dissatisfaction with results
  • Infection- though unlikely, infection of treatment sites is a risk of treatment. It is rare that an infection becomes complicated, however if you suspect an infection has occurred please contact your artisan immediately. Signs would include redness, heat, pus and pain that does not improve.
  • Sensitivity/allergy response, anaphylaxis. Though rare, allergy to PRP, hyalauronic acid slip, topical anaesthetic and/or any of the agents used in the process of treatment can occur. If you are concerned please contact your artisan.

+ How many treatments will I need?

Often some improvement is seen following one treatment, however a course of 2 or more may be recommended depending on the severity of the skin condition being targeted. Re-treatment is optimally performed at 3-4 weeks, with maintenance after the initial course often required.

+ What happens during treatment?

During treatment you will discuss the risks and side effects of treatment with your artisan; prior to being provided the opportunity to ask questions and sign a consent form. Photos will be attended, and the treatment area(s) identified.

This area will be prepared for treatment and in most cases topical numbing agent applied.

The skin will be cleansed with chlorhexidine to decontaminate prior to treatment, which will be proceeded by applying a motorised pen to the skin and a hyalauronic acid slip.

Following treatment the site will be re-cleansed and post-treatment cream + SPF will be applied. A soothing, cooling and hydrating mask in addition to LED light treatment may be purchased as part of a more comprehensive skin package.

+ Is it painful?

Skin needling is only mildly uncomfortable when performed with topical anaesthetic, and this is generally isolated to the forehead and bony areas of the face where tissue tends to have less skin density. Topical anaesthetic is applied routinely prior to treatment and often no additional pain relief is required.

However, paracetamol may be also be taken within 30 minutes of treatment if you are not on medical advice to avoid this medication. We do not however recommend aspirin, diclofenac (voltaren), meloxicam, ibuprofen (neurofen) or any other anti-inflammatory prior to treatment as these are likely to excerbate swelling and bruising, and decrease the efficacy of treatment.

You are likely to experience some sharp discomfort during treatment, however please advise your artisan of any intense pain.

+ What should I do prior to treatment?

  • DO NOT WEAR MAKEUP TO YOUR APPOINTMENT
  • Reschedule if you develop any active skin conditions/infections/inflammation at the site of treatment
  • If you are prone to hormonal pigmentation it might be recommended that you commence a course of skin preparation for 2-4 weeks prior to treatment
  • Consider paracetamol 30 minutes prior if your doctor has not advised otherwise
  • Cease topical retinoids and chemical exfoliants for 72 hours prior
  • Consider commencing on arnica to assist with reducing swelling and bruising following treatment
  • Avoid microdermabrasion, chemical peels, laser therapy and any treatments that impair skin integrity for 2 weeks prior to treatment

OVERVIEW

Chemical peeling is application of varying concentrations and types of acids to the skin. This may help to remove the superficial layers, and force skin to regenerate while deeper peels may also help to stimulate collagen. Chemical peeling is suitable for treating a number of skin conditions, with efficacy dependent on the depth, peel solution used and frequency of peeling.

At Aescend we use professional/prescription-grade chemical peels from skin care brands 'Medik8' and 'Synergie'.

While most are suitable for chemical peeling, it may not be recommended for anyone pregnant or breastfeeding, those with salicylic sensitivities, active infection, immunosupression or a history of cold sores and/or keloid scarring.

PDO MONO THREADS OVERVIEW

Mono threads are made from a material called polydioxanone (PDO), an absorbable polymer often found in sutures. When inserted superficially into the skin, mono threads may add support and structure. They may also help to stimulate collagen, providing additional smoothing and tightening effects. Over a period of 3-6 months, patients might experience thicker, plumper tissue, as well as improved lines and wrinkles. As such, mono threads are frequently used for rejuvenating the eye area and treating lip lines, the nasolabial folds, jowling, accordian lines, and neck lines. They are useful for improving tissue laxity, scarring and stretchmarks.

+ How many treatments will I need?

PDO threads help to create new collagen that supports and tightens tissue with effects lasting 6-12 months after treatment. Longevity of results may vary depending on lifestyle factors such as smoking, alcohol, exercise etc. A second treatment may be recommended at 6 months to maintain the initial tightening effects.

+ What should I do prior to treatment?

  • DO NOT WEAR MAKEUP TO YOUR APPOINTMENT
  • Factor bruising and recovery into your planning. It is recommended that treatment be performed at least 2 weeks out from a special event
  • Consider paracetamol 30 minutes prior if your doctor has not advised otherwise
  • Consider commencing on arnica 2 weeks prior to treatment to reduce downtime

  • Consider cessation of smoking
  • In consultation with your doctor, consider ceasing any blood-thinning medication including fish oil and anti-inflammatories at least 2 weeks prior to treatment
  • If you have a history of cold sores, preventive medication is often recommended and is essential in cases of frequent or severe outbreaks
  • If you have any active wounds, infections or outbreaks please delay treatment until healed

The skin will be cleansed with chlorhexidine to decontaminate it prior to treatment, and multiple small needles will be inserted under the skin. This will last 10-30 minutes depending on the treatment area(s).

Following treatment the site will be re-cleansed and a topical antiseptic cream applied. Ice and/or LED light treatment may be recommended to reduce downtime and expedite recovery.

+ Is it painful?

The procedure to insert threads is uncomfortable, requiring multiple skin penetrations and may not be recommended for those with severe needle phobia. Topical and local anaesthetic may be used at the time of treatment to improve comfort after which no additional pain is often required.

However, paracetamol may be also be taken within 30 minutes of treatment if you are not on medical advice to avoid this medication. We do not however recommend aspirin, diclofenac (voltaren), meloxicam (mobic), ibuprofen (neurofen) or any other anti-inflammatory prior to treatment as these are likely to exacerbate swelling and bruising.

You may experience a firm feeling and some tenderness in the area that has been treated for a few days after the procedure, and twinges are likely in highly mobile areas i.e. around the mouth.

+ Aftercare

Prior to leaving the clinic, your artisan will provide you with printed aftercare information, however generally following treatment you should:

  • Avoid intense exercise or any activity that increases blood pressure and/or body temperature so as to avoid exacerbation of swelling and/or bruising; and reduce risk of infection. Saunas, spas and swimming should be avoided for at least 3 weeks
  • Avoid touching the treated area and/or applying any makeup to the skin for 12 hours. Where possible avoid oil-based products for 24 hours and in the 5 days following treatment approach skin care regimes (including make up application) with care.
  • When applying makeup do not use brushes/sponges for 2 days due to cleanliness of the brush/sponge.
  • Paracetamol and ice may be used for discomfort once anaesthesia wares off
  • Sleep with head elevated, on your back until resolution of swelling
  • Avoid direct sun exposure and wear spf 30+ daily
  • Avoid use of physical exfoliants or active skin care at the area for at least 72 hours
  • Avoid dental surgery for 4 weeks
  • Avoid exaggerated facial movements like chewing, talking or yawning for 3 weeks, instead keeping facial movement within normal expression
  • Avoid chemical peels, microdermabrasion, or intense skin treatments/cosmetic intervention for at least 3 weeks. Light and laser based treatments or treatments like radio-frequency and HIFU (ultrasound) that apply heat to the tissue should be discontinued for 3 months following treatment.
  • Hirudoid cream, arnica (topical or oral) and bromelain may be used post treatment to expedite recovery
  • In rare circumstances you may experience thread extrusion after treatment, simply pull the thread out and cover the wound with Bepanthan

COG lifting threads are moulded strands of polydioxanone (PDO) polymer which when inserted superficially under the skin may help to lift/reposition tissue and encourage lay down of collagen.

COG lifting threads may help to restore volume, improve sagging and deep folds, and gradually tighten and rejuvenate skin. Some immediate improvement is often seen following treatment, however maximal results are not expected for up to 6 months following treatment, with secondary tightening often seen at 6-8 weeks.

+ What are the risks and expected side effects?

Immediately following treatment:

  • Bleeding, bruising, swelling and redness is common, with bruising and swelling potentially lasting upto 14 days. In most cases this can be covered with cosmetic products. Alternatively we are able to offer interventions to assist with the resolution of bruising + swelling
  • Skin may feel tight and slightly tender
  • Asymmetry is likely while ever there is swelling
  • Bumps and/or inflammatory nodules may be felt under the skin and may take several days-weeks to resolve
  • Skin dimpling/pulling is common and may persist for 3-4 weeks. Although alarming it is best to allow this to settle for a more superior lift. Some correction can often be achieved via gentle manual massage and/or with resolution of swelling
  • Temporary facial paralysis from local anaesthesia is likely and occurs due to treatment of the facial nerve during the numbing process. Although alarming it is most often transient, resolving in 2-3 hours.

Less commonly the following adverse effects may occur

  • Temporary/long term changes to pigmentation may occur as a result of skin inflammation though may be managed with additional interventions available in-clinic
  • Haematoma (collection of blood) may occur due to damage of blood vessels. This should be identified and drained (where necessary) early to avoid complications of tissue breakdown and infection. For tenderness, discolouration, swelling or bogginess of the tissue that does not improve, please consult your Aescend artisan.
  • Allergic reaction to the threads or one of the products used during preparation. If you experience inflammation, redness, pain, swelling at the treatment site(s) or are concerned you are having an allergic reaction please consult your artisan. For symptoms such as mouth/lip swelling, difficulty breathing, dizziness, chest tightness, please visit your nearest emergency department.
  • Delayed healing and/or infection is possible without adherence to good after care practices. During your treatment we take the utmost precautions to ensure asepsis and prevent infection. Aftercare information should be followed carefully to ensure infection is not introduced after your procedure
  • Extrusion of threads which may require re-intervention is possible though uncommon. Threads may wriggle themselves superficially with time and/or excessive movement. This may lead to them becoming palpable at the ends. The best intervention is to leave them and allow them to break down with time.
  • Damage to deeper structures such as nerves, blood vessels and/or muscle can occur during insertion though is unlikely. If concerned, consult your artisan.
  • Scarring is possible though unlikely in the absence of infection and/or a predisposition to hypertrophic/keloid scarring.
  • Loss of lift COG/barbed threads are designed to be a temporary treatment with results lasting 1-2 years. While we endeavour to carefully select patients and carry out a technically proficient procedure, there is no guarantee of satisfaction with results, nor duration of effect which may be less than the time stipulated.
  • Asymmetry and dissatisfaction with results. Treatment is not a precise process and is limited by naturally occurring asymmetries/variations in physique. Treatment may not meet your expectations, nor provide the results you were hoping for thus we will never guarantee satisfaction
  • Local anaesthetic toxicity. Is a very rare side effect of local anaesthetic administration and is unlikely at the doses used during treatment. Manifestations are usually within 1-5 mins after injection with onset between 30 s - 60 mins. Symptoms may include metallic taste, light headedness, dizziness, visual/auditory disturbance, tinnitus, disorientation, drowsiness, chest pain, difficulty breathing, profuse sweating, palpitations and shortness of breath. For symptoms that persist please notify your Aescend artisan and/or attend your local hospital for any emergencies.

+ How many treatments will I need?

COG threads help lift tissue and create new collagen that supports results for 1-2 years after treatment. Longevity of results may vary depending on lifestyle factors such as smoking, alcohol, exercise and aftercare practices etc.

+ What should I do prior to treatment?

  • Consider rescheduling vaccinations to more than two weeks either side of treatment
  • Reschedule if you develop any active skin conditions/infections/inflammation at the site of treatment
  • DO NOT WEAR MAKEUP TO YOUR APPOINTMENT
  • Factor bruising, puckering and recovery into your planning. It is recommended that treatment be performed 6 weeks out from a special event
  • Consider paracetamol 30 minutes prior if your doctor has not advised otherwise
  • Consider commencing on arnica to assist with reducing swelling and bruising following treatment
  • Avoid alcohol consumption 48 hours either side of treatment
  • Consider cessation of smoking
  • Cease any blood-thinning medication including fish oil and anti-inflammatories (in consultation with your doctor) at least 2 weeks prior to treatment
  • If you have a history of cold sores in the area, preventative medication is often recommended and is essential in cases of frequent or severe outbreaks
  • If you have any active wounds, infections or outbreaks please delay treatment until healed
  • Avoid any cosmetic treatment or deep exfoliation such as dermal fillers, chemical peels, facials or skin needling, for two weeks prior

+ What happens during treatment?

During treatment you will discuss the risks and side effects of treatment with your artisan; prior to being provided the opportunity to ask questions and sign a consent form. Photos will be attended, and the treatment area(s) identified.

The area will be aseptically prepared, and your skin cleansed with chlorhexidine to allow decontamination prior to treatment. Topical anaesthetic will be infused along thread tracts to assist with comfort.

The procedure itself will involve insertion of several large cannulas which act as introducers for the threads. These cannulas are removed and in doing so deposit the thread into tissue in the areas discussed in your treatment plan.

Following treatment, the sites will be re-cleansed, and steri-strips applied over the wounds. LED light treatment may be used to aid fast recovery, prior to fitting you with a head garment.

+ Is it painful?

The procedure to insert threads is only mildly uncomfortable. Local anaesthetic is utilised at the time of treatment to improve comfort, with often no additional pain relief required.

During the process of instilling anaesthetic, you may experience sharp, stingy pain. This is due to the acidity of the anaesthetic and is only fleeting. During insertion of the threads the tissue should be numb though you might feel pressure, pulling and tugging. Sometimes during this process break-through discomfort may be experienced.

Pain should however dissipate quickly and may be replaced with a slight 'bruised', 'tight' feeling.

+ Aftercare

Prior to leaving the clinic, your artisan will provide you with printed aftercare information, however generally following treatment you should:

  • Expect mild tenderness to persist for approximately 1-2 weeks. Paracetamol may be used as required, though anti-inflammatories such as ibuprofen and aspirin are not recommended as they are likely to exacerbate bruising and swelling.
  • Apply ice packs 10 mins on, 20 mins off to help reduce swelling which will also be alleviated by wearing your head garment and reclining on a few pillows to sleep following treatment.
  • Leave steri-strips in place until they fall off or apply topical antiseptic e.g. chlorsig ointment or bepanthan over the insertion points twice daily for one week (as advised by your artisan).
  • Expect bruising as an unfortunate side effect of treatment. LED, arnica (or hirudoid) cream and/or bromelain may help to expedite recovery.
  • Allow puckering to settle, this may take 3-4 weeks though will eventually happen. A greater lift is achieved by leaving puckering to resolve
  • Avoid oil-based moisturisers/makeup for 48 hours and ensure the face is treated gently (especially during skin care regimes and makeup application) for at least 2-4 weeks following treatment as it is possible to break threads with excessive tension. Avoid sponges and brushes during application of makeup to reduce risk of infection during this time.
  • Try to avoid sleeping on the treated area(s), massaging the area(s), applying direct pressure at the sites where the sutures were placed and drinking through a straw for 2-4 weeks. Following treatment contact sport, high intensity exercise and dental surgery are not recommended for 4 weeks. Care should be taken to avoid excessive/exaggerated facial expression and movement following treatment e.g. chewing, yawning, talking, smiling. Normal expression is fine.
  • Wear your facial garment for 2-3 hours daily for 2-4 weeks following treatment.
  • Avoid any cosmetic treatment or deep exfoliation such as chemical peels, facials or skin needling, for two weeks prior, and four weeks following treatment unless discussed with your artisan
  • Avoid laser and heat therapies including radiofrequency, IPL and ultrasound modalities 3 months post treatment to ensure longevity of results.
  • Avoid active/harsh facial products (AHAs, BHAs, vitamin A/retinoid) for at least 1 week post treatment.
  • Avoid alcohol for 24 hours following treatment.
  • Apply 30+ sunscreen following your treatment and reapply as necessary in the days after treatment

OVERVIEW

Light therapies use light in the visible and non-visible spectrum to alter the function and structure of various cells/cellular components. These actions can be harnessed cosmetically where they may improve the appearance and health of skin and alter the growth of hair. Within the umbrella of 'light therapies' are Intense Pulsed Light (IPL) and Light Emitting Diode (LED).

IPL machines emit filtered wavelengths of light and deliver them under energy . Various wavelengths penetrate different layers of the skin and may selectively target different pigment molecules known as chromophores. These chromophores include melanin (pigment in hair and skin), haemoglobin (pigment in blood vessels) and P acnes (the bacteria that causes acne). When exposed to the respective wavelength, a process known as photothermolysis occurs, and selective destruction of pigment molecules may be achieved. IPL is therefore indicated for treating redness and visible blood vessels on the face, brown pigment caused by sun-ageing, acne and permanently reducing unwanted hair.

On the other hand, selective wavelengths of light are passively emitted during LED treatment. The exposure alters the action of different tissue cells, effecting various biochemical processes such as wound healing, collagen formation, inflammation, pigment formation, activity of P acnes bacteria and hair growth.

light therapies (IPL)

LED TREATMENTS

OVERVIEW

Light emitting diode (LED) is technology originally harnessed by NASA to encourage growth of plants in space.

In a cosmetic sense the technology has been associated with wound healing, reducing inflammation, inducing collagen production and manipulating the activity of bacteria.

Different wavelengths of the light spectrum (blue, green, red, infrared, yellow) penetrate the skin at different depths and may affect biochemical processes in tissue thereby altering cellular function.

BLUE - light may reduce the activity of the sebaceous glands and may also be absorbed by P acnes bacteria. Blue light may help in the treatment of acne and help to expedite healing of third degree burns.

RED - light is thought to stimulate fibroblasts (cells that produce collagen) and may therefore improve healing, and the signs of ageing. Red light may also be useful for reducing inflammation and treating inflammatory skin conditions such as eczema, psoriasis, rosacea and dermatitis. Via its proposed effects on collagen, red light may improve skin hydration, the appearance of fine lines, muscle recovery and pain. Furthermore, red light has been linked with hair growth promoting benefits, potentially by improving the health of tissue and aiding blood flow.

light therapies (LED)

LED TREATMENTS

OVERVIEW

GREEN - light may help to reduce the activity of melanocytes (pigment producing cells) thereby reducing the appearance of brown spots/freckles. It may also influence symptoms of depression through the release of serotonin. By influencing serotonin it is believed that green light may also influence hunger and appetite and thus have positive weight management effects.

INFRARED - light works similarly to red light, though is proposed to have the ability to penetrate deeper into tissue and influence lymphatic drainage. Thought to stimulate energy production in cells, IR may expedite recovery from swelling, bruising and inflammation.

LED is a treatment option, that is suitable for all skin types and most skin conditions. There is no preparation involved and rarely any downtime. We use LED as an adjunct to many of our in-clinic treatments, offering it as a free service to help with bruising following procedures.

  • For treatment of unwanted hair, it may take upto 3 weeks for the body to shed the treated hair. Do not pick/pluck during this time

Less commonly the following adverse effects may occur:

  • Skin Tone changes (light or dark spots on the skin) lasting 1-6 months
  • Freckles may temporarily or permanently disappear in treatment area. Other potential risks include crusting, itching, pain, bruising, pimple-like bumps, dry skin, hypopigmentation (lightening of the skin), hyperpigmentation (darkening of the skin), blistering, burns, infection, scabbing, swelling and scarring
  • Failure to achieve the desired result
  • Allergic or delayed inflammatory reactions - can develop though are very rare
  • Eye injury - protective eyewear must be worn during treatment.
  • Dysplasia/hypo-pigmentation of suspicious lesions - prior to initial treatment, any sun spots should be correctly identified by a physician as benign and suitable for light-based treatment.

+ How many treatments will I need?

Depending on your treatment, multiple sessions are required. During your initial consultation, your artisan will determine the number of suggested treatments. This is dependent on factors such as your skin type, area of the body being treated and the target chromophore. Generally hair removal can be performed at intervals of 4-8 weeks, and skin treatments at 2-4 weeks.

It is also recommended that you return for more than one rejuvenation treatment as this will ensure you experience maximal results. That said, improvement is often visible following initial treatment.

+ What should I do prior to treatment?

  • Avoid exposing the area to the sun or applying fake tan in the two weeks prior to treatment
  • Consider paracetamol 30 minutes prior to IPL if your doctor has not advised otherwise
  • Cease use of vitamin A/retinol based skin care in the area at least 2 weeks prior
  • Avoid alcohol consumption 48 hours either side of treatment
  • Consider cessation of smoking
  • Cease any photo-sensitising medication, blood-thinning medication (in consultation with your doctor) at least 2 weeks prior to treatment
  • Shave the treatment site approx 2-3 days prior to hair removal *note an additional fee will be incurred for any hair removal sites, not appropriately shaven prior to presentation

This area will be prepared for treatment and you will be provided safety googles to protect your eyes from the light. Your artisan will also don eyewear and other personal protective equipment (PPE). The machine settings will be entered by your artisan and you will be given the opportunity to re-feel the therapy prior to proceeding.

Treatment will proceed and your artisan will continue to enquire as to your comfort. During treatment you will see the flashing of the light through your protective googles, and potentially smell the plume of hair being treated.

Following treatment the site will be re-cleansed. You will be offered ice for comfort if required and post laser cream + SPF.

+ Is it painful?

IPL is somewhat uncomfortable for most, likened to a hot, flicking rubber band. Topical anaesthetic may be used for the face to reduce discomfort, however in darker skinned individuals this may encourage hyperpigmentation. Ice is available if required following hair removal and paracetamol may be also be taken within 30 minutes of treatment. We do not recommend aspirin, diclofenac (voltaren), meloxicam, ibuprofen (neurofen) or any other anti-inflammatory prior to treatment as it is likely to exacerbate swelling and bruising and may encourage burning.

You are likely to experience warmth during treatment however please advise your artisan if pain exceeds 6/10. Often IPL for skin treatment is described as 'a flicking rubber band' on the skin.

+ Aftercare

Prior to leaving the clinic, your artisan will provide you with printed aftercare information, however generally following IPL:

  • Apply post laser cream, aloe vera or a bland moisturiser to the treated area until symptoms resolve
  • Avoid friction on the area from clothing etc until healed
  • You should avoid adding excessive heat to the area (saunas, hot showers) and or submersion of the area (pools, spas) for at least 1 week post treatment
  • Apply ice as required for comfort
  • Sleep with head elevated until resolution of swelling
  • Avoid direct sun exposure and wear spf 30+ daily
  • Avoid excessive exercise for 24-48 hours following treatment
  • Avoid use of physical exfoliants or active skin care at the area for at least 72 hours
  • Avoid chemical peels, microdermabrasion, IPL/laser, or intense skin treatments/cosmetic intervention for at least 2 weeks

VASCULAR LASER

x close

Downtime and pain with the Vasculaze vascular laser is similar to that of our other light and laser therapies such as IPL and Tixel with patients expecting:

  • mild redness (erythema), burning (sunburn sensation) and swelling for 1-3 days (longer with more aggressive treatments)
  • microcrusting and/or bruising that can last 5-7 days (longer on the lower extremities)

If you are interested in treatment with the Vasculaze vascular laser please book for a free consultation or following the link for treatment here

Pre-treatment preparation is as per our other light and laser therapies and includes: abstaining from sun exposure and tanning products for 2 weeks prior, ceasing topical retinols, acids and anticoagulants for 7 days prior to treatment (on medical advice), shaving any dense hair in the treatment area.

Q-SWITCHED LASER OVERVIEW

A Q-switched laser is a non-invasive laser capable of treating to the deep dermis. Our Q-switched laser is an ND:YAG laser that emits wavelengths of light at both 1064 and 532 nm.

Unlike other ND:YAG lasers used for ablative therapy and/or hair removal, the Q-switched laser delivers its energy via a series of nano-second pulses. This makes it suitable for treating tattoos, as the high intensity of the laser beam disperses tattoo pigment within the dermal layer of the skin without allowing tissue to accumulate heat. The same process is able to stun pigment-producing melanocytes without causing excessive tissue disturbance, and in this way can be employed as a treatment for pigmentation/melasma also.

By painting liquid carbon on the skin and letting is sink into small skin depressions, we can also perform the carbon 'hollywood' 'china doll' facial with this laser. Using a larger handpiece to selectively ablate carbon off, non ablative resurfacing may be performed. This may assist with exfoliation, and treatment of pores, fine lines, dullness and irregular texture.

q-switch laser FAQS

  • BLACK PIGMENTED TATTOO REMOVAL
  • RED + ORANGE PIGMENTED TATTOOS
  • CARBON LASER FACIAL
  • PORES + FINE LINES
  • SKIN REJUVENATION
  • TREATMENT OF PIGMENTATION + MELASMA

+ AM I SUITABLE?

Most people are suitable for treatment with the Q-switched laser however you may be declined treatment in the setting of the following:

  • pregnancy and breastfeeding
  • cancer or chemotherapy sensitivity and compromised immunity
  • concurrent (or recent) use of roaccutane and/or light sensitising medications
  • diabetes and/or conditions that affect healing
  • autoimmune, peripheral vascular disease or bleeding disorder
  • tanned or sunburned skin in treatment area
  • renal failure
  • active infections/outbreaks at the site
  • multiple sclerosis or epilepsy
  • surgical metal pins or plates under tissue to be treated
  • pacemaker in situ
  • allergic reaction to ink
  • history of keloid scarring and/or darker skin types
  • heavy smoking and/or alcohol use

+ What are the risks and expected side effects?

Immediately following tattoo removal:

  • The site will have a 'frosted appearance' this should resolve over the next few hours and the tattoo may return to the original colour
  • A change to pigment colour is expected - this includes lightening/darkening of pigment and/or a change to red, black, peach and/or light brown
  • Swelling, bruising, redness and weeping are commonly expected reactions. Pinpoint bleeding and swelling will usually resolve within 72 hours
  • Blistering and crusting may occur post treatment. Do not pick or pop these, instead ice blisters and keep any broken areas clean, moist and protected with bepathen or solugel
  • Transient textural changes to the skin may occur though should resolve within a few months of treatment
  • Photoallergic reaction and/or reaction to liberated tattoo ink may occur following treatment. Advise your artisan for any signs of itching, redness, swelling etc that arise additional to the intitital treatment reaction

Following carbon facial and skin rejuventation:

  • Some warmness and redness may remain in the skin, this should be non painful and will resolve quickly
  • Generally skin will look immediately more vibrant, with tone, texture and pores visibly improved

  • Allergic or delayed inflammatory reactions - can develop to any of the products used during the process of treatment, or following liberation of ink from tattoos. These are however, very rare. Following carbon facial, incidences of post-treatment histamine release have occurred, during which patients can experience a transient, itchy, red rash at the treatment area.
  • Eye injury - protective eyewear must be worn during treatment.

+ How many treatments will I need?

Tattoos require numerous treatments 6-8 weeks apart, however as they fade, your artisan may recommend extending the time between treatments to facilitate ink resolution and minimise unwanted side effects. A consult is required to determine the number of suggested treatments, as this is dependent on factors including ink colour, depth of tattoo, degree of scarring, tattoo location and where on your body the tattoo is.

Carbon facials and skin rejuvenation treatments may require less treatments and can be performed at intervals fo 2-4 weeks. It is recommended that you return for more than one carbon facial and/or skin rejuvenation treatment as this will ensure you experience maximal results. That said, improvement is often visible following initial treatment.

+ What happens during treatment?

During treatment you will discuss the risks and side effects of treatment with your artisan; prior to being provided the opportunity to ask questions and sign a consent form. Photos will be attended, and the treatment area identified.

This area will be prepared for treatment and you will be provided safety googles to protect your eyes from the laser beam. Your artisan will also don eyewear and other personal protective equipment (PPE). The laser settings will be entered by your artisan and you will be given the opportunity to re-feel the laser.

Treatment will proceed and your artisan will continue to enquire as to your comfort. During treatment you will see the flashing of the laser through your protective googles, and hear the popping of the sound that it makes.

Following treatment the site will be re-cleansed. For tattoo removal you will be offered ice for comfort, antiseptic ointment and an occlusive dressing will be applied.

+ Is it painful?

Tattoo removal is generally an uncomfortable procedure and may result in bleeding and breaking of the skin. Unfortunately because tattoos are targeted deep in the dermis, most topical anaesthetics do not provide much relief. Ice is the best form of comfort, however paracetamol may be also be taken within 30 minutes of treatment. We do not recommend aspirin, diclofenac (voltaren), meloxicam, ibuprofen (nurofen) or any other anti-inflammatory prior to treatment as it is likely to excerbate swelling and bruising.

Carbon facial treatment and/or skin rejuvenation are quite comfortable and you are likely to only feel mild warmth applied to the skin with each emission of the laser.

+ What should I do prior to treatment?

  • Avoid exposing the area to the sun or applying fake tan in the two weeks prior
  • Consider paracetamol 30 minutes prior to tattoo removal if your doctor has not advised otherwise
  • Shave the treatment site approx 2-3 days prior to tattoo removal

+ Aftercare

Prior to leaving the clinic, your artisan will provide you with printed aftercare information, however generally following tattoo removal:

  • Apply aftercare cream (solugel or bepanthen) and dress the area as recommended by your artisan
  • Avoid popping any blisters or picking any scabs that form
  • Avoid friction on the area from clothing etc until healed
  • You should avoid adding excessive heat to the area (saunas, hot showers) and or submersion of the area (pools, spas) for at least 1 week post treatment

Generally following treatment you should:

  • Avoid direct sun exposure and wear spf 30+ daily
  • Avoid excesive exercise for 24-48 hours following treatment
  • Avoid use of physical exfoliants or active skin care at the area for at least 72 hours following carbon facial
  • Avoid chemical peels, microdermabrasion, IPL/laser of intense skin treatments/cosmetic intervention for at least 2 weeks following carbon facial
